Complaint

ReklamacjaService request should contain the following information:

1. Name and model of the wheelchair

2. The serial number (stamped on the frame or read the warranty card)

3. Date of Purchase 

4. Name and Surname of the owner

5. Mailing address and contact information (phone, email)

6. Damage - an accurate description of the fault along with photographs

Properly completed form please include as an attachment and send in the manner described above, together with a receipt or invoice. All service orders execute only on the basis of confirmed reports on our form. In the email please provide necessary and sufficient information reported by your problems. If you have any problems with describing a fault, please contact us. You can also use postal mail or fax, however, please take into account that in this case the delivery process of your order will be significantly longer. After receiving the form, and become familiar with your problem will pass on information feedback on possible further remedial activities, the date and the possibilities of positive solutions.
